Appointment Information
Please have the following information available when scheduling the appointment and bring it with you to the appointment.
- Patient’s Name
- Patient’s Date of Birth
- Insurance Card (Subscriber or Insured’s Information)
Insurance and Other Payment Options
Many employers change plans on an annual basis, so we will ask for your insurance card/information when scheduling your appointment and at each visit to be sure our records are accurate. This helps ensure that your claim will be filed and paid with as little inconvenience to you as possible. We offer a variety of other payment options. Dates/Times
Medical appointments are generally available Monday through Friday from 8:30am to 4:30pm. Cosmetic appointments are generally available Monday through Friday from 8:30am to 6:00pm.

Cancellations and Rescheduling Policy
Please contact us at your earliest convenience, if you are unable to make your scheduled appointment. This courtesy will allow our office to make the appointment available for other patients. We thank you, in advance, for your cooperation.

A minimum of 24 hours notice is requested for cancellation or change.
Cosmetic Appointments – 214-751-2388
A minimum of 48 hours (2 business days) notice is required prior to the appointment for cancellation or change. Otherwise a no-show fee appropriate to the scheduled service will be charged to your account.
